The project aims to enhance the well-being and quality of life of older people in Tonga by providing appropriate, safe, and high-quality integrated health and care services, and support to caregivers. For the first time, Tonga will implement a multifaceted approach to aged care, recognizing that aging involves physical, emotional, social, and psychological aspects. The project will also support caregivers (who are predominantly women) to promote a more sustainable and positive experience in providing care for older people and a psychosocial approach to care. Climate change adaptation is effectively built into the project activities. The project is aligned with the following impact: a higher quality of life experienced by older people in Tonga. The project will result in the following outcome: integrated aged care system improved and expanded.

Project Rationale and Linkage to Country/Regional Strategy Tonga's aging population is a significant social and economic challenge. The country's inadequate aged care system cannot meet the growing needs of older people. Without early interventions to promote healthy and active aging, issues like noncommunicable diseases, social isolation and loneliness can contribute to a rapid deterioration of health and well-being among older people, which can strain healthcare and social resources. Care for older people is primarily provided by unpaid family caregivers, most of whom are women who also lack support and lose the opportunity to be economically productive. The burden of caregiving can affect caregivers' quality of life and the quality of care they provide to older people. The project will address these issues.

Summary of Environmental and Social Aspects
Environmental Aspects The anticipated environmental impacts of the project's civil works component are site-specific and localized, and the works are not expected to have significant or irreversible negative environmental impacts. The following conditions apply to civil works contract awards: (i) the Tonga Department of Environment has granted final approval of the IEE and updated EMP, indicating whether environmental permits are required; (ii) the EMP requirements and provisions for environmental management are incorporated into the bid documents for civil works and construction contracts; and (iii) MIA has obtained ADB's clearance of the IEE based on final design and it is in compliance with ADB's information and disclosure and consultation requirements. MIA, through the PMU safeguards personnel, is responsible for project environmental safeguards management, monitoring, and reporting. Capacity will be enhanced with additional training.
Involuntary Resettlement Long-term leases will be required for three parcels of land. A resettlement plan has been prepared following the laws of the Government of Tonga and ADB's Safeguard Policy Statement. Ministry of Internal Affairs, through its PMU, will ensure effective implementation and monitoring of the resettlement plan.
Indigenous Peoples Due diligence has confirmed that the project will not impact any indigenous people groups as defined under ADB's Safeguard Policy Statement. The project will ensure that local communities actively participate and receive culturally appropriate benefits.
Stakeholder Communication, Participation, and Consultation
During Project Design The project design has been informed through wide consultations, including survey and focus group discussions with older people and caregivers. About 724 Tongans aged 65 years and older were surveyed and interviewed, and a separate survey was undertaken with 307 caregivers. All key government agencies (e.g., Ministry of Finance, Ministry of Internal Affairs, Ministry of Health, and Ministry of Infrastructure) and NGOs were consulted. Communities located near the sites of the aged care centers were also consulted to receive their feedback about the project and avoid any social and environmental impacts.
During Project Implementation The project will continue to consult with community members, churches, and NGOs during implementation. Women and households, particularly older people and their caregivers, will be specifically invited to attend focus group discussions to ensure their concerns and priorities are considered in all activities. Various community activities will be implemented to promote volunteerism and support by young people to care for older people. The project will also coordinate with non-government organizations.
